We ask our patients to pick a primary care provider in the Mountain View Network to help coordinate your care. They will work closely with you to make sure you get the preventative care you need to keep you healthy. If your health puts you at risk of needing to stay in the hospital, your primary care provider will also work with you directly on providing case management services to ensure you get the proper treatment and care.

In addition to the doctors who care deeply about your well-being, Mountain View Hospital’s Population Health team may also help to improve the coordination of your care. This team of clinical and non-clinical employees may reach out to you or your doctor to discuss your health concerns in order to help guide you to a better outcome as quickly as possible. The Population Health team may also contact you directly regarding referrals your provider has made. This is to ensure all providers, including your primary care physician, are informed of the visit and get a complete picture of your health. The Population Health team was created to support your health needs and direct you to resources inside the Mountain View Network, so you can reduce what you pay for your medical care.

We always suggest you carry your health insurance card with you when you go to the doctor, so the staff is aware of your coverage. There are several different plans that include the Mountain View Network and each plan has slightly different benefit limitations. It’s important the doctor’s office knows you are participating in a Mountain View Network plan, so they make in-network referrals to keep your costs down. All provider office policies differ so please check with your provider’s office to verify they accept your policy.

It is important to us that you build a relationship with one doctor, so they can have a complete picture and understanding of your health. Sometimes, patients need to see providers that specialize in certain areas of healthcare. When you go and see a specialist, it’s not often that the results of that visit make it back to your primary care provider. We want to fix this. We know how important it is that your primary care provider understands everything about your medical history, so they can truly help you be the healthiest version of you possible. Having a primary care physician who knows you will eliminate unnecessary testing, medication conflicts and ensure the best care for you and your family. It’s important to select your primary care provider as soon as possible, so you can start to build a strong relationship with them.

You might pick a primary care provider and realize it wasn’t the perfect fit for you. That is ok. The Mountain View Network allows you to change your primary care provider throughout the year. You can designate a new primary care provider on the Blue Cross of Idaho website or through Mountain View Hospital’s Population Health team by 208.542.7032. If you switch doctors, the change will take effect on the first of the following month.
